Your Rights
Patients have the right to respectful care, clear information, privacy, confidentiality, and the opportunity to ask questions about care options, risks, benefits, and next steps.
Participation in Care
You are encouraged to participate in decisions about your health, tell the clinic about your symptoms and concerns, and ask for clarification when instructions or plans are unclear.
Privacy and Records
You may ask about how your personal health information is handled and may request access to or correction of your health record, subject to applicable legal requirements.
Your Responsibilities
Please provide accurate and complete information, bring your health card and medication details when needed, attend appointments on time, follow agreed care plans where possible, and notify the clinic if you cannot attend.
Respectful Environment
Patients, visitors, clinicians, and staff are expected to treat one another with respect. Harassment, discrimination, threats, or abusive behaviour may affect access to non-urgent services and may require further action.
